Prom Guest Approval Form (Required for Non-JMRHS Guests)

If your student plans to bring a guest who does not attend JMRHS or Cabarrus County Schools, this form must be completed and approved before a ticket can be purchased.

Deadline: Friday, April 10
Completed forms must be turned in to Mrs. Rodriguez in the Main Office.

Guest Requirements (must meet one of the following):

Option 1: Current High School Student
• Under age 21
• In good standing at their school (administrator signature required)
• Parent or guardian permission required
• Homeschool students must provide a valid photo ID

Option 2: High School Graduate
• Under age 21
• In good community standing
• Must provide college or university information and/or employment information
• Must submit a copy of a valid photo ID

Additional Information:
• The guest must be approved before the JMRHS student can purchase a ticket
• Forms will be reviewed by administration and the Prom Coordinator
• Guests must agree to follow all CCS, JMRHS, and venue expectations

Important: Forms that are incomplete or submitted after the deadline will not be approved.
